Hello,Let's assume that your website is my-company-domain.com and you want to use customerForms.my-company-domain.com for your forms. Here is what an example form would look like:

Step 1. To use your own domains in Jotform, add a DNS record:
Add a CNAME record which points to custom.jotform2.com with your subdomain. Here is what it will look like in the DNS:
