I want a single address on the top of each page. Is there a way I can do that without calculated values?

Unfortunately, that is not possible. One form element cannot be connected to multiple areas in your PDF. So, the only option is cloning that element, and passing the value from the original element to the clone one: https://www.jotform.com/help/351-how-to-automatically-pass-form-data-to-another-form

Using two forms complicates the process I was trying to create. Is there any way to do it with only one form?

Sorry, I provided the wrong guide. I meant to say passing the value from the original field to its clone(in the same form): https://www.jotform.com/help/286-how-to-pass-field-value-to-another-field?
