I have done several threads, but hadn't gotten an answer. We upgraded to premium yesterday and today were deactivated. Reference #

I need to get this form back on. Please manually reactivate the form.

Good day! Thanks for creating a new thread and for providing the Order Reference Numbers. I've found the payment, and I have now manually upgraded your account to our Premium Monthly Plan. Please kindly check again.
Our system has failed to automatically upgrade your account because you haven't define your Jotform Username during the process.

Thank you for upgrading the account. Just so you know the upgrade form doesn't have a line for user name. It asks for email address only.
