Approval Form

I have started this form, but I still can't seem to get my approval emails to send correctly.

I need 1 email to send to {supervisorEmail} upon completion of the form to approve/deny. And then I need an approval/denial email to send back to the person requesting the leave.

Ideally - I would also get a notification upon approval. Any ideas?
