why is my form deleted

 why was my form https://form.jotform.com/201204107197041 deleted?

Can it be recovered? How do I prevent this from happening again? We are interested in upgrading to the Silver Package for HIPPA compliance when we have our form put together.

It seems the form belonged to another account, as I cannot find a historical record for any form deletions or form purges on your account EyeCare5

Do you happen to have another account? If so, please let us know and we will try to locate it, but if the form was purged from the trash folder manually or after 30 days, there is no way to retrieve it
