Submission Notification is missing data

    We have been using jotforms since the pandemic hit, in March and it really has helped our medical office but now we are having SEVERAL issues that really makes us rethink using you. These issues slow us way down and that is just something we cannot have right now be a major covid testing site.

    • When some people submit their form it just comes over as "submission ###" therefor not getting all the request information.
    • It takes ATLEAST 2-3 minutes just for jotform to load. NO this is not an internet, browser, or computer issue. We have verified all this is good with out ITs and the only website we have issue with is jotform.
    • We request patients to upload ID and insurance card on some of our forms. Most of the time the patients either cant upload because they get different error messages or if it does it will duplicate one of the pictures so we only get one item and not two.
    • Patients have issues completing and submitting their form. On the very last page they will either get an error that they did not complete a field, which they did, or when they submit that is an error message. Sometimes we do get their form and usually if they get that error we get the form multiple times. Some patients even redo the paperwork multiple times and it will never submit to us. This is a huge waste and frustration on our patients fault

    As a paying customer we expect more from jotforms. These forms are a huge part of our daily operations and it really is frustrating and stressful on us as well as our patients. Like I said previously, we use to never have an issue and now the issues are daily.

    We love our forms and we find that jotforms was the easiest and most convienent site to use but again, if we cant get this figured out we will need to switch to a different company that we will have less issues.

    I really hope you take this matter seriously and assist us.

    Can you please clarify this? Are you receiving the Notification emails with only "submission ###" as the content? Please note that most of the fields on your form will be hidden in the notification email due to HIPAA compliance on your account.
