I am not receiving our patient forms

    I just checked the last few submissions received by the form and it matches the last email notification that was sent from it:

    You can also check these logs on your account: how-to-view-all-your-form-email-history.

    Also, please check if the email is in your spam/junk folder. If it's there, mark it as NOT SPAM so that it will not be flagged as spam in the future.
