I want the form to be filled out by people without an account.

I have sent the link to one of my forms to a co-worker and to myself via email. When either my co-worker or I go to open the form, all we see is a blank white screen. Neither of us is logged-into the JotForm account when trying to open the form. We want to send these forms to people without a JotForm account, so we want to make sure they can be opened and filled out by people without an account.

Please let me know what can be done so the form can be opened.
