Files are not properly uploaded

We request patients to upload ID and insurance card on some of our forms. Most of the time the patients either cant upload because they get different error messages or if it does it will duplicate one of the pictures so we only get one item and not two.

For users getting an error message, they need to click on the upload button and wait for it to complete. Otherwise, just loading the file won't send the attachment to submissions.

As for the duplicate part, I wasn't able to replicate it. May I know if you are able to replicate it on your end?
