The form conditions are not working as expected

    I have the condition logic set for only Which type of service is this request for? under the Service Plan column to show questions 11 - 16 if Delivery Service is selected but its displaying those questions without it being selected, is there a reason why?

    I see there are several broken conditions on your form. Could you please fix those conditions:

    If after fixing those conditions it is still showing the fields without selecting the options then add another condition to hide the fields when the field Which type of service is this request for? is left empty.
