What happened to my data?

    It seems that much of my data for one of my forms has been lost or deleted. I checked the Activity Log and see a series of deletions that I did not do; some of which took place on days that none of my team wasn't even in the office.

    I went ahead and changed my password in case someone was able to break into my account but how do I figure out how this happened so it doesn't happen again and is there a way to retrieve this data?

    Hello Marc - We are unable to login into your account to check due to HIPAA restrictions. As you can see, even support from Jotform can not access your data or submissions due to HIPAA security.

    Perhaps, someone has access to your account? You can check the login history in the account activity logs and see if you can find an unfamiliar IP address there.

    Please try to regain the submission data in the submission email. You can review the submission sent email of your form in your sent email history logs.
