Ordering multiples of same item but different sizes

How do I set up my form (in which I am not collecting a payment), for people to order a t-shirt where they may want 1 size small, and 2 size mediums. Right now it only allows you to select one size.

In your product sizes options, you have to turn on the "Create Sub Products from Options" for the sizes as shown below.

Oh gosh, I looked and looked and looked for this! I didn’t think it would be under the sizing option, it makes sense now, but boy did I try and look on my own!
