I need to get my account reactivated

The logo was made by Farm Bureau because I'm a contracted agent with them. But just go ahead and remove it so my account can be activated. Thank you

I did check your account and found that it is currently suspended because of the copyright logo you have added in all your forms. Please check the screenshot of the logo:

Are you authorized and have permission to use this copyright item in forms? Please send us proof that you are authorized to use this logo. 

Since your JotForm account profile does not show an email address related to it, this is violation of our Terms of Use. As you have already mentioned to removed form, I have deleted the logo images from your forms.

Please change the account profile email address to one that matches the domain with logo, and then you can add the logo images in your forms again.
