Why is the form main title displaying incorrectly?

Hi,

We have 5 forms all of them are showing up as Driver Application when the title is different. I even checked the source code in iFrame and it is correct. For example, service mechanic has the correct title in draft mode but when you hit preview or view the embedded code on our website it does not display as 'service mechanic' it displays as 'Driver Application.'

As I check your JotForm on the web page, I notice that the welcome page on the form has not been modified. You may either edit the welcome page on the form or remove it. Please see the screencast below for help.
