I cloned and tested the form and I was able to fille out and successfully submit the form:
Can you please use a different browser or a different device to make the submission? Perhaps in a private window or incognito mode?
I will also clear your form cache so let us know how it goes.

Yes the problem is not consistent and this was using the mobile app not a browser? -

Hi there,
I've created a clone of your form and it seems to be working as expected on the mobile app as well. The error is occurring on a specific device or on random devices?
