    I reviewed the terms. The credit card authorization form I created I am not going to use. I realized after that I could not use it without the paid service. That form can be deleted. The other forms are to capture client request for travel.

    Hello Leigh - As checked, your account has been suspended as this form: Credit Card Authorization Form Fillable, aside from collecting credit card details on plain text fields, contains a Copyrighted Logo. It is considered as a scam and violates our Terms of Use.

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your forms are used.
