    My account was disabled when I was creating a credit card information form. After researching I realized I need to use the payment forms you have already created. Please delete this form and reactivate my account. Also, can you restore the previous form I deleted called Release of Information Form. Thank you for your help!

    I checked on the account/username elliecentralorcounseling and I noted that it was auto suspended by the Phishing detector due to the suspected form that violates the Terms of Use.

    You have form that collect credit card information using plain text fields.

    Due to our PCI certification, we are required to deny the collection of credit card details on regular forms.

    However, we have other PCI-compliant ways to do this. All you need to do is choose any of the payment processors available in our platform in order to integrate in your forms. You still won't be able to collect credit card details directly though.

    If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of a feature called "Payment Authorization". To learn more, kindly follow this guide on How to Enable Payment Authorization.

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the suspected forms.

    I have now deleted the suspicious form and reactivated your account. You can check the updated status when you log in again using the username at https://www.jotform.com/login

    You can still restore the deleted form in your My Forms Trash folder.
