Account deactivated

Hello! I am just trying to delete this form because I want to use this address with my other account. However, it's apparently deactivated for phishing? And I'm not sure how that happened, since all I've done is make printable PDFs out of Google Forms for my students...but whatever. All I need to do is delete this please! Thank you!

We apologize for inconvenience. I checked the account/username kerisalyards and I found it was auto suspended by the Phishing detector on April 2020 due to suspicious form.

However, I checked the form and the account just now, and it appears to be a mistake.
