Hello, I am creating a form for patients to be able to make an appointment for covid testing.
Each location will have different hours associated, how can I create a form that changes the available appointment times based off which location you select?

Currently, my locations are listed as a dropdown menu, and I add in the appointment widget. Is there a way that the appointment widget changes per each location in the dropdown menu?

    Thank you!

No, it's not possible. What you can do is add multiple appointment field and add a dropdown field with location as the selection.

Now, create a condition to show and hide the appointment field is location is equals to..

And to answer your question, do you mean you want the selected location to be added to the message body of the autoresponder email? If so, I believe that's added by default. Just ensure you have disabled the PHI setting for the location and appointment fields so they will be shown in the email. Here's a guide on How-to-set-PHI-fields-on-your-forms.
