Can you please explain to me why the confirmation email does contain the scheduled time or the reserved time slot when I have it set up to do just that?

The unique name placed in the autoresponder is correct. I also sent a test submission to your form and the email I received contained the date of the appointment slot.
Can you please test the form again and let us know if the issue persists on your end?
