incorrect form linked

    I'm still having issues with incorrect forms linked!! This is ongoing and not resolved.


    This link https://form.jotform.com/202068778015861 is linking to the 'NEW CLIENT CONTACT FORM' (correct link for this is  https://form.jotform.com/19355694866407) when it should be linking to the postnatal initial form.


    please help - i have contacted you several times and it still hasn't been resolved!

    When I open your form with URL https://form.jotform.com/202068778015861 here is what I see.

    I do not see the new contact form on this URL.

    The second form URL you provided is not working. The form has been deleted already.

    Please create a short video where we can see the new contact form opening on the Post Natal Pre Screenshot form URL so we can further investigate the issue.
