Form calculation widget: calculation was missing after cloning the form

    Hi guys,

    Have been working on the form: https://hipaa.jotform.com/build/202438575317862

    with numerous calculations which were all tested and working fine, then randomly every single calculation field has been wiped from the form. I have tried putting it back to a previous version but they are still wiped/turned off.. has a setting on the form been changed or something to block/remove calculated fields? Please help this took hours.

    To clarify, your form calculation condition has been removed and you have used the revision history to revert your form but the issue still persists?

    Hi Richie,

    Best way to explain - I duplicated this form: https://hipaa.jotform.com/202438575317862 (the Somnocare Sleep Questionnaire) which has a bunch of calculations (if you can see the actual online form you will see them). Then built out the new questionnaire (Somnocare Patient Questionnaire) with more calculations etc. where the bug has occurred (https://hipaa.jotform.com/202931538590862). The screenshots below are from the PDF report where you can see the top image has all the calculations, then not long after putting through that questionnaire, I submitted a new one (bottom screenshot) where no calculations have taken place. When I went back to the online form, literally every single calculation field was turned to off, and all the calculation widgets were cleared (if you have access to the back-end of the Somnocare Sleep Questionnaire you will see a bunch of calculations - these should all be there in the Somnocare Patient questionnaire plus more). So yeah, in the space of 10 minutes between the first survey being submitted and the second of the Somnocare Patient Questionnaire, all the calculation fields in the survey just spontaneously cleared... Then I reverted back to old versions and the calculation fields and widgets all have no calculations worked in either so completely perplexed.

    Hi there,

    I can see that the calculation widgets do not have any calculations in them. However, when I create a clone the calculations widgets do get cloned with all the existing calculations in them. I would suggest that you create another clone and use it instead.

    As far as the results in the PDF are concerned I've noticed that your PDF attachment is not password protected. Please note that in HIPAA enabled account the PDF attachment must be password protected. Otherwise, it will not work properly.

    Hi Vick,

    I just tried cloning the "Somnocare Patient Questionnaire" and the calculation fields and widgets are all still blank. Did you mean clone the "Somnocare Sleep Questionnaire" again? If so, I would have to redo hundreds of questions and branching rules that I added after originally closing the Somnocare Sleep Questionnaire to make the Somnocare Patient Questionnaire. It would be quicker for me to just re-do all the calculations directly into the "Somnocare Patient Questionnaire" than duplicate the Somnocare Sleep Questionnaire again and spend hours recreating all the additional questions.... If I have to do this, so be it -- but it is really concerning that it randomly wiped all this in the first place and what's to stop it happening again? There doesn't appear to be an option of copy/pasting hundreds of questions including their branching rules from one form directly onto another to avoid having to re-do them all..

    May we know if the PHI field on the original form calculation widget was enabled prior to cloning or did you enable it in the cloned form?

    The only thing I can think of doing that I did before it broke was get it to send Email on completion with a password.
