    I have signed up via Google and my sign up is no longer working

    Online Form Builders, including JotForm, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting ID numbers and displaying copyrighted logos on the forms is considered as phishing activity, as stated in our Terms of Use.

    Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

    If your purpose is not to collect these data from the general public, but from your own customers, then you need to comply with two things.

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, social security cards, driver's licenses, credit/debit cards and statements, bank statements, voided checks, birth/marriage certificates, visas and photo IDs.

    As for the copyright logo, in order to reactivate your account, kindly provide us the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    You are requested to send these proofs to reviewers@jotform.com with Thread #2651092 as your subject. We will let you know about the status of your account after a thorough review of your proofs.
