The questions "How old are you?" and "Are you a US Citizen?" are always showing up on my form even though the conditional logic I have set for them says not to show unless another action has been taking. These 2 questions have an image associated, which are respectively "Image - 3" and "Image - 14".

Can you help me figure out what is wrong and how to fix it?

That's due to condition priorities which conflicts other conditions. You should try re-constructing those conditions that's called twice. Try moving the other condition on top of it if you'd like to prioritize a condition. You can also create another condition and put it at the top of all the conditions. Like if a field is not filled, hide "How old are you?" field. Honestly I am having a little difficulties understanding your conditions. I'm not sure as to why you're hiding a field once it's filled in, I'm afraid it won't capture any data anymore since the filled field is hidden thru condition.
