    Can I store patient Information securely in your system (electronic forms/notes)?

    Can your customers create templates for daily notes, evals, consent to treats, and have the ability for patients to sign these documents?

    Is there a customer service number to call?

    Please note that the JotForm forms are secured by default and also HIPAA compliant. If you are required to make the forms HIPAA compliant, you can upgrade to Silver and Gold plans: how-to-upgrade-to-HIPAA-compliance?

    Can your customers create templates for daily notes, evals, consent to treats, and have the ability for patients to sign these documents?

    Yes, you can create these types of forms. You may also refer to our template gallery if you need to look for templates in building a form: https://www.jotform.com/help/106-how-to-use-form-templates.

    Then, for your users to sign or update the form after submission, you would need to include the Edit Link of the form on the autoresponder email: how-to-let-users-update-their-form-submissions-at-a-later-date
