How do I add to a form that I submitted for my employer?

Do you want to edit a response that you entered in a form?

If yes, you will need to contact the form owner directly and ask them to make the change.

Otherwise, you could have done it from the end, if there was an edit submission link provided in the confirmation email that you received after form submission.
