I am not entirely sure if this can be done. From what I knew, restoring forms take some effort and time to accomplish. I will have this escalated to our higher level support department. We'll see what they can do about this.
Just in case your form can be restored, we would need more details to make restoration easier (if possible). What is the direct link to the form? Can you give the exact date and time when the changes were made?
Also, for your information, it is possible to disable autosave in your forms. Refer to this user guide to learn how to do it.

The date the form was changed was 5/16/2011. Anything from 5/15/2011 prior would be still correct. I am unsure of any other link to provide you other than the http://www.jotform.com/form/1721647280 address.
The issue was not with an autosave problem. For some reason IE8 was unknowingly truncating the bottom portions of our forms (the problem did not seem to present itself in Firefox 4). We made a change using IE8 and saved without seeing that the bottom portion was not visible.
Thank you for your help!!!

While I do not think that IE8 has any control over your form in edit mode, much weirder things happen with browsers, so I cannot disregard that possibility.
It is most likely though that the missing form fields were accidentally deleted. Anyway, like I said, our higher level support personnel will have to take a look into this before any action can be made. Thank you for your patience.
