Remove the ban on my account

    Please remove the ban on my account.

    I was planning to use Jotform for a new website but will have to use another vendor if you can't unblock me by tomorrow.

    If there is an issue with a phisher using my test form, then just disable that form and notify me, don't block my account.

    Your account was suspended because your form was suspected to be offering a fake service. If your company is a legit institution and you don't want us to delete the form, you will need to provide the following information:

    1. The URL where the form is used.

    2. Provide any document that will support your claims.

    3. Your website or proof of affiliation to a legitimate institution.

    Send all of these to reviewers@jotform.com with "Thread #2654647" as the email subject. One of our colleagues will get back to you about the status of your account.
