You can check this cloned form that I made http://support.jotform.com/form/32495280746966. The user will be required to choose "Skilled pay grades" and "Unskilled pay grades" once done if user will click on submit button it will redirect user to paypal page.

Here a guide on how to clone a form. Feel free to copy the form that I made.

Let me know if you are ok with this.

really and truly it is eye sore. i mean really we cant do any nicer than that, like get the apply now button to act as submit. i actually really would prefer a drop down menu exactly like i had from pay pal where the portion seen is the skilled 40.00 and the drop down is the 30.. i definitely want the 40 to be the choice that is obvious. even the text is like from a typewriter and really just looks thrown together.

Please check the other thread you have: http://www.jotform.com/answers/265328. I have you the same output per your Paypal's Code. You just need to clone the form back and use it instead of your current form.

If you don't like the form and if you'd like that output to be made to your form, please let us know and give us authorization to do so.

Also, we request you to please do not create a separate thread for the same inquiry. This way, we can assist you better without having to check the other threads you have and for us to avoid confusions about the status of your inquiry
