Any ideas for a form I want to create

I run a business where I service farm operations. My idea for my road crew team was to create a form that has a map of the operation and all the details of what needs to be done. A checklist of some sort. Is there a template similar that you would recommend I start working off of? I would love to have map That I would make so I can upload it and add more pics of important areas to address. Then on the bottom have checklist to make sure what they need to get done. I would love for them to open that as a reference. Not necessary a form for them to fill out but to open it to have as a guide. Thanks in advance

Hello! You should be able to achieve this with our tool. You'll need to implement a workflow that would require you to set operations (checklist) for your crew.

When you fill and submit this form, your crew will receive an email with a link to this form that you filled.

They'll be able to open the same form from their web browsers, view all the tasks and can then check/uncheck the task they complete; after this they can submit the form. They can open and submit this form as many times as they wish to.
