Spreadsheet to Form Widget: Not Pulling Information

We use JotForm as a way to register customers for sale events. We attach a spreadsheet to the form and when you put in the 5 digit PIN code for that specific customer and click the auto populate button it says the form has been filled, but there's no information there. This seems to have just happened and has never happened before. What could the problem be?

I have cloned your form, upload a test spreadsheet, and it works fine. 

Please make sure the value you are inputting in the Pin Code field matches the form Access code column. We have a more detailed guide here: How-to-Use-the-Spreadsheet-to-Form-Widget.

Please give it a try and if the issue persists, kindly provide us a screenshot of the excel file so we can further check.
