Form has the same PDF Document as the template it was created from

I am using on of the other forms as template and incorporating changes, although the form looks different, on submission the pdf is the one from the

If I'm understanding your concern correctly, you are referring to the PDF Document that is associated with your form. You can update and customize the content of the PDF document to your preference by going to the PDF Editor page of your form.
