Request integration with Omni Fund

We are a wellness program that handles reoccurring payments for multiple veterinary hospitals. We are wanting to be able to design forms to send to clients for each hospital to encourage enrollment in program before client arrives. Client fills out form and then when returned we would have accounting department contact client for credit card info to be entered into our portal with special coding on credit cards with Omni fund.
We work with about 200 clinics currently and are growing. We could work with one user password to design forms and we just want to be sure as we set up clinics that each clinic can have individual link to form with clinic info, due to each clinic has its own plan per clinic.
So this is why we want to be sure we pick the correct plan to work for what we need.
Any suggestion on best option you have would be appreciated. We have been looking at deferent platforms like DocuSign and Hellosign , so why would your program be better for what we are needing.

Since you want to customize the form for each clinic, please note that you can build a separate form for each clinic which you can customize based on your requirement. You can then grab your form's direct URL and share it with your clinic. Please note that each plan has different limits including form limits. Please take a look at our pricing page for more information on available plans and its limits: https://www.jotform.com/pricing/

If you work with 200 clinics and want to build one form per clinic, you need Gold plan which has unlimited form limit.

We do not integrate with Omni Fund as for the moment, but I've sent this request to our backend team, if there are more people requesting the same feature, our engineers might consider the integration for the near future.
