    No, it didn’t change. It’s supposed to Have collapsible sections and this
    looks the same as it just was.

    Our apologies for the inconvenience. I checked your form and all those 3 changes last Thursday is logged at 23:55. I tried to revert to the first and the last version for that day but the categories were not carried over.
