Spreadsheet to form widget is not working

Having similar issues as gilesoh. Spreadsheet populated fine last week and now no fields are completing (but no error message). Please advise.

Can you please share the form with the issue and share the Spreadsheet to form unique code so that we can test it out?

I have tested our sample spreadsheet to form widget and it seems to be working.
