My page break is broken. It's not properly inserting a page break where it should

    My form is http://www.jotform.com/form/11395418799

I'm using Firefox and the form seems to not be functioning properly where I have my first page break.  As you can see, it's inserted a question below the page break where there should be nothing.  It should just be a button to go to the next page.  Help?

Upon thorough inspection and multiple tests, I have found out that the culprit for the messed up page break layout is the <u></u> tags you have inserted in the 'Program Setup' radio label.

HTML <u> or underline tags are already deprecated and is seldom supported.  I have removed those tags from that particular label and replaced it with <font style="text-decoration:underline"></font>

Please do confirm if your form is working fine now. Thank you.
