Hi,
I have a form that I have set up to auto populate with the spreadsheet to form widget. It was working fine for a while- yesterday what started happening is when the people trying to complete the form input the unique code, they get the 'form filled out successfully' message, but none of the answers actually fill out- the entire form is still blank.
Our account is pepofaz and the form I'm having issues with is Annuals (Connected to Roster, EMR Order)
I've tried cloning the form, deleting and re-adding the widget, and re-uploading the spreadsheet, but nothing seems to make a difference.

I checked the form and the spreadsheet to form widget is functional on my end:
We did receive a few other reports of the widget not being functional but in each case clearing browser cache or clearing form caches did allow for the widget to be functional again:
