Sorry to bother you but am I missing something.

I have created a form but want to be able to email the form to customers to complete and return but can't see how I save the form to outlook as a template rather than send a link to customers to fill in online.

    Is this possible?

    Hello @CliveWilliams,

Thank you for contacting our support team.

Unfortunately, it is not possible to save your form as a template on Outlook. if you are looking to send your form to your customers for it to be filled, you can simply take the Form URL and send it via email and your customers will be able to use the form via the link. I assume you are referring to your Appointment Medical and Consent Form, correct?

It seems that this form is currently set to Private mode which restricts the form from being accessed by anyone that has not been assigned by you. If you want the form to be accessed by anyone, you can change the privacy settings on the form to public or company access. OR if you want to keep the form private, you can simply assign the form to the specific people that you want to fill it.

Unfortunately, I am not able to replicate the issue you are mentioning. I am able to open the form on my end without any issues.

Can you kindly share the link you are sending here with us so we can check it?

    Thank you for sharing that.

Since the form is no longer private, you do not need to share the "assign link". Please use the regular form URL that is located in the Quick Share Tab.

That's all working now but just a few more issues and thank you for your patience.

Using ASM select rather than one drop down box per question I need multiple drop downs for one question.

When receiving the completed form the signature does not display?
