Autofill widget is not working.

I use an autofill widget in one of my forms and it is giving either a string error at entry 345 or claiming it is filling in entries without doing so.

Is there something I need to do to get this working again?

I can't seem to find the widget added to any of your forms.

Could you please tell us which form are you referring to?
