If you have a PDF form, you can import that into JotForm as explained here: https://www.jotform.com/help/548-how-can-i-import-my-own-pdf-with-smart-pdf-forms

Once you map the fields correctly and publish the form, users can fill and submit and receive a copy of the data in your PDF form after form submission.
