HIPAA Compliant Telehealth Forms

Hello,

We are looking for a way to send patients HIPAA compliant forms for their Telehealth visits. We do not do many telehealth appointments maybe 5 per month. Do you offer this and if so how does the process work and what is the cost? We need something fairly easy for patients to access and sign. Thank you!
