We use your service to collect patient information, but are not sure how it is HIPAA compliant if we send this information to a 3rd party. Can you help us understand what you mean that jotform is HIPAA compliant.

Hello Kyle,
I assumed that you are referring to this form: 202735253928156, correct? If it is, I can see that it is under the jborie account, and as I have verified the account, HIPAA is not enabled on that account.
If you are referring to a different account, kindly provide us with the username of the account or the email account, to verify the information on our end? You can enable HIPAA on your account if you are in the Silver or Gold plan, and this can be done through the Account's Data page:
