    There is no phishing intention or attempts by the existing jotforms that we created in development. We are mindful of the misimpression that one may have with seeing username and password fields being designed together, but that was in fact meant to be the fabrics of the login box of our in-house membership system. We shall change the relevant design to avoid any risk or perceived Image of phishing or other identity hacks. Please therefore reactivate our account.

    I deleted the password field from your form and activated your account. Please avoid adding similar fields to the form.
