Hello my name is Charles Dean. I was at one time receiving response emails, however I am no longer receiving them. I am not a tech person and do not understand your "Add an Email" layout. On the bottom it has my email already added, however I am not receiving them. I have done several test and none of them come to my email address which is email@example.com. The form that is on my site is "Contact" form

I have checked your form and cannot find a Notification Alert being set up? I can only see an AutoResponder which is different.
Please go to Setup & Embed:
1. Click Emails Alerts
2. Choose Notification Email
3. Configure the settings, and set recipient email to: firstname.lastname@example.org
