i found that in several computers and smartphons the widget is not working well:
it finds the record in the spredsheet a green check mark is apearing but
the fields in the form are not being filled as before.
it started around three days ago.
on some devices i instructed them to change the webexplorer to chrome or microsoft expolorer... and it worked.
but in others i couldn't solve it.

There was an update for the Spreadsheet to Form widget. Please clear the browser cache and check again.
