If you wish to create multiple dropdowns, you might want to use the Dynamic Dropdowns widget.
With it, the values of the next dropdown will be shown depending on the chosen value of the first dropdown. You can learn more here:

Thanks for the quick response but what I am looking to do is rather than check the box the answer is yes or no
The question is "Do you suffer with any of the following.........

Hi CliveWIlliams,
With a dynamic dropdown widget, you can only select 1 option per dropdown. It does not currently support multiple selections.
You can use the Multiple Selection Widget instead to allow multiple selections then show/hide it through the condition.
