Hi,
We are selling a sleep test and after the purchase we need to collect some private data from the client. Can we embed a form on the thank you page and collect that data and have it stored on your servers?
Thanks
Ron

Hi Ron,
Happy to help!
You can absolutely embed a form on an external webpage, and you can make the form multilingual.
First, let's cover how to make your form multilingual:
1) In your Form Builder, clicking the SETTINGS tab on the top menu. Ensure that the FORM SETTINGS tab is select, and then click the SHOW MORE OPTIONS button:
2) Scroll down to Form Language and set the language you would like your form available in. For the purposes of this example, I have set the form language to English. We'll add the Hebrew translation when we click EDIT:
3) Click Add form language:
4) Search for the language, and then click on it when it appears in the dropdown. Then, click ADD FORM LANGUAGE:
5) You will need to translate any untranslated fields. Then, you can click the HIDE FILLED ENTRIES button as you are translating to hide the fields that have already been translated. Finally, check the FORM WARNINGS tab to ensure that you have also translated all form warnings:
Once translated, navigate back and you will see the language added. Please note that you can Set Primary if you choose:
Please note that, when embedded, the Hebrew translation may read left to right instead of right to left. To correct this, you can inject the following CSS code into your form:
.form-all[dir="rtl"] .form-header,
.form-all[dir="rtl"] .form-label {
text-align: right !important;
}
Here's our guide on How-to-Inject-Custom-CSS-Codes
Second, let's cover how to embed the form:
1) In your Form Builder, click the PUBLISH tab in the top menu. Click on </>EMBED and then select the embed code that you'd like to use:
As you can see, there are multiple ways to embed your form, and picking the correct one can make a difference. Here's our guide on how to pick the correct embed code: which-form-embed-code-should-i-use
Also, I see that you had said that you are collecting patient details: please note that JotForm does offer HIPAA Compliance. Here's some more information on how to become HIPAA Compliant and everything that becoming Compliant means: https://www.jotform.com/hipaa/
I hope this helps! If we can help further, please don't hesitate to reach out.

I'll respond to your other question on a separate thread https://www.jotform.com/answers/2665065 to avoid confusion.
