I want to ask some questions and if answered correct customer will receive a pre approved notice and if answered incorrectly they will receive a notice that states need to further look into this for a pre approval so that we get all data

You can certainly create a pre-approval form process using Conditional Logic; however, before I start with my example, I do want to mention that we currently have several form templates available from users that have pre-approval conditional logic as well.

I've created a screen recording of a demo pre-approval form to show how conditional logic works:
1) First, insert the form fields that you would like to include, including fields that will show for either situation (pre-approval or rejection). Make fields required as needed.
2) I would first recommend setting up your Thank You Page. I would set this as a Thank You page for those who have been pre-approved. Access the Thank you page via the SETTINGS tab in the top menu, and then click on THANK YOU PAGE:
3) Then, set a new Thank You page for those who have been rejected as shown in the example above. Please note that I had uploaded a caution symbol to the images on my own and that this symbol is not readily available: you will need to upload it yourself.
4) Continue to set your conditions as shown in the example above. Ensure that you have named your emails appropriately to select the proper email to send to the proper recipient. Again, I have shown how to do this in the example above.
Please ensure to familiarize yourself with our Terms of Use prior to asking for any sensitive information: asking for Social Security Numbers, credit card details, etc. are in direct violation and can automatically trigger our phishing detector which will results in account suspension.

Not sure how to create a second thank you page and still not sure on the not approved. The conditions. I still want them to answer all the questions so we can see which ones caused the denial.

Have you had a chance to watch the first video in my previous answer yet? It gives an overall view of each condition and the process as a whole.
You can set a second Thank You page (in this case: for those who are not approved) like so:
Also, through Conditional Logic, you can choose not to hide fields if you'd like the respondent to fill out all forms. In this case, you would remove any Show/Hide Conditions for the form fields.

Here is our guide on Adding-a-form-to-your-wix-site: you may need to adjust the width/height of your element within Wix after you add the form, but Wix is typically good at adjusting as necessary.
