How can I duplicate a form I like?

    Hello:

New user. I'm attempting to duplicate a form used by a similar agency and trying to figure out how they did a couple of things.

First, we - like them - need to gather certain pieces of information for every member of the household. Is there a quicker way to gather name, DOB, work status, etc. than just duplicating every field and creating a condition that makes them all appear?

I understand - and appreciate - conditional fields, but it looks like they may have used a widget. Wondering which one - there are A LOT!

This is time sensitive so I don't have a lot of time to slog through webinars - although I plan to after this tight deadline is met.

The form I'm modeling on can be found here: https://mvcap.jotform.com/201406151042134

It sure looks like they have used a widget for the "Additional Household Members' section.

We are also a Community Action Agency given the same grant funding. Just trying to model our form on theirs.

    Many thanks!

    Hello,

Thank you for contacting JotForm's support!

I have inspected the form you are modeling on, and I noticed that they use the "Configurable List" widget, with custom CSS rules applied to adjust the display of the list.

You could add this widget to your form too and set it up as you want.

Moreover, if you want to save time, I suggest that you clone the form you are modeling on. Simply copy its URL and import it in our Form Builder!

Let us know if that helps,

    Thank you!
