The above given form is not displaying properly on few of our office IOS Ipads.
Since we use IPADS to get the forms filled from the patients.

When you say it is not displaying properly, do you mean it is cut off at the bottom or the margin/padding is incorrect? Can you clarify which part is not displaying properly?
The form was embedded as an iframe, I see no reason it would fail to display.
If the issue persists, please try embedding using our default embed method (https://www.jotform.com/help/34-Embedding-a-Form-to-a-Web-Page).

I tried your webpage in the IPADS simulator, but I was unable to replicate the issue. Can you please tell us what IPAD model you are using so we can test it with the same?
Also, please show us a screenshot of your webpage when you open it on the IPAD that is not showing the form.

Please find the attached image showing the same page on two different ipads.
The form appears on 1 ipad but doesnt appear on other 2 ipads

Can you try the default embed?
I see no reason why the form won't show up since it's currently embedded as an iframe which is useful for preventing script conflicts.
